Primary Care 24 (PC24)
Primary Care 24 – previously known as Urgent Care 24 - delivers a range of NHS urgent and primary care services covering 1.3 million residents across the whole of Merseyside. Based in Liverpool, we provide patient services around the clock. Our established GP out-of-hours service has adapted well to the new integrated urgent care requirements and continues to evolve as a high quality, multi-disciplinary service. At PC24 we deliver daytime general practice across 12 GP practices with a combined population of 40,000, along with the Enhanced Access Service to approximately 750,000 patients. We support the provision of the Clinical Advice Service, 24/7, where patients contacting NHS 111 with a pre-defined condition(s) and who would usually be referred to AED, are now referred to us for further assessment. We provide health screening and medical assessments to refugees and asylum seekers who have recently arrived in Liverpool, and we also provide clinical support to patients who have been temporarily admitted to intermediate care beds in nursing homes.
