Addressing Health Inequalities: A project that has identified that certain patient groups are not getting equivalent health care and has taken steps to improve delivery of care to that group. It demonstrates an understanding of how health inequalities come about, has worked to understand some of the barriers that the group experiences and produced an effective action plan that addresses the issues.
