Urgent Care 24 is the established local provider of GP Out of Hours Services to the residents of Knowsley, Liverpool and Halton with a patient population of approx. 750,000. They believe that delivering services across the whole of the 3 CCGs provides an exciting opportunity to transform patient care. They have provided out of hours services locally since November 2004 and have gained invaluable knowledge and experience of local patient demands and are proud of their record of compliance against the National Quality Requirements.
Their GP-led model of care has attracted national attention and has influenced service provision in other areas. They have developed informal support networks and contacts at a national level to share best practice. Their approach to quality is rigorous and they are supported in delivering a quality service by their stable and highly skilled workforce. They have developed a greater understanding and experience of providing Primary Care Services since 2009 to the Initial Accommodation Asylum Seekers who arrive in Liverpool.
Urgent Care 24 along with Liverpool Community Health NHS Trust & Knowsley Integrated Provider Service formed an innovative partnership to provide primary Care services to NHS Knowsley for those patients who traditionally have difficulty accessing a GP, this is known as the Options Service.
They are part of a Community Intermediate Care Bed Pilot; the pilot consists of GPs visiting homes, supporting the multi-disciplinary teams, prescribing and referring patients for further diagnostics if required, thus ensuring that patients are not re-admitted back into hospital.
